Using images of children

It is the custom and practice of this School, to take still and moving images of children for use in displays,
newsletters, school publications and for promotional purposes. Images may be used internally or externally,
in printed format or electronically, including online and in social media. We ask all parents to complete the
attached form detailing the various preferences for their child. We will not disclose the full name of any
child alongside still or moving images at any time.
From time to time, the media who will take photographs or film footage of a visiting dignitary or other high
profile event may visit our school. Pupils will often appear in these images, which may appear in local or
national newspapers, or on televised news programmes.
To comply with the Data Protection Act 1998, we need your permission before we can photograph or make
any recordings of your child. Please answer questions 1 to 5 below, then sign and date the form where shown.
