On occasions your child may be asked to taste certain foods in relation to topic work/festivals/celebrations
etc. We have therefore decided to obtain food allergy information for each child in school and ask that you
complete the form below. This way we can be sure that your child is not asked to taste anything that he/she
is allergic to. We do ask however, that food allergies
ONLY
are listed and not items that your child may just
have a particular dislike to.
